Isolated abdominal aortic dissections are rare events. Their anatomic and clinical features are different from those of atherosclerotic aneurysms. We report a case of isolated abdominal aortic dissection of Saudi female aged 39 years presented to our hospital with abdominal pain and she was diagnosed as a case of Marfan’ syndrome with isolated infra-renal abdominal aortic dissection. In management of this case we verified that the surgeon must has a high index of suspicion in Marfan’ syndrome patients who present with abdominal pain and hypertension for possibility of abdominal aortic dissection. Abdominal ultrasound as initial imaging modality followed by computed tomographic angiography are recommended to diagnose the case and aortic reconstruction using a graft is often successful.
