Objective; To assess the incidence of undiagnosed intrauterine pathology based on screening with office hysteroscopy in women with normal hysterosalpingogram (HSG) and/or transvaginal ultrasonograghy (TVS) Design: Comparative Study Setting: AL-Shatby University Hospital, Alexandria, Egypt Patients: one hundred and eighty consenting patients were eligible to participate in the study, divided into two groups, 90 patients in group I were indicated and prepared for ICSI for their first time and 90 patients in group II were indicated and prepared for ICSI but with previous history of failed ICSI before. Main outcome measures: Unsuspected uterine pathologies. Results; Unsuspected abnormal uterine findings were diagnosed in (36.7%) of patients with normal HSG and/or TVS among patients in group I and in (38.9%) of patients in group II using officehysteroscopy. Conclusion(s):The incidence of pathologic abnormalities based on hysteroscopic diagnosis was high especially with repeated IVF failure. So, routine office hysteroscopy should be an essential step of the infertility workup before ART even in patients with normal HSG and/or TVUS.
