THE EFFECT OF BETA-3 ADRENOCEPTOR AGONIST, ALPHA BLOCKER AND ANTI-MUSCARINIC ON URETERIC STENT RELATED SYMPTOMS

Introduction: Although the beneficial effect of stents in urology. They are associated with stent related discomfort (frequency, urgency, and pain) which have an impact effect on the quality of life. Objective: To detect the effects of beta3-adrenoceptor agonist, alpha 1 blocker and anti-muscarinic on JJ stent-related urinary symptoms as regard efficacy and safety. Patients and Methods: A prospective study was conducted on 80 patients from March 2017 to March 2018 at Al-Zahraa University Hospital. Patients were randomized divided into four groups, Group I: received placebo once daily, group II: received anti-muscarinic (solifenacin5mg) once daily, group III received alpha 1 blocker (silodosin 8mg)once daily, group IV: received beta3- adrenoceptor agonist (mirabegron 50 mg) once daily. All patients were assessed for side effects, bothersome lower urinary tract symptoms (LUTS), pain and quality of life at pre-operative day, post-operative days 3 and 14. Results: There were no statistically significant different at pre-operative and post-operative day 3 between groups (I-IV), at post-operative day 14 there was statistically significant different between groups, (LUTS) were less in groups (II-IV) (P< 0.05), mirabegron improved frequency more than solifenacin and had no dry mouth which expressed with solifenacin, there was a significant reduction in nocturia and pain with silodosin. Conclusion: This study showed that beta3-adrenoceptor&anti-muscarinic were the best medication in symptomatic patients following JJ stent insertion. Mirabegron improved frequency more than Solifenacin and had no side effect of it. Silodosin improved nocturia and pain.
